DFG BA877/17-1 Die photopische negative Antwort als Biomarker für die funktionelle Integrität des Sehnerven

For rational treatment of glaucoma it is important to detect the disorder at the earliest possible stage. To date, a multitude of techniques is used to diagnose glaucoma making use of functional (e.g. perimetry and pattern electroretinography) and morphological (e.g. scanning laser polarimetry, scanning laser tomography, optical coherence tomography) measurements. However, the predictive value and sensitivity of these measurements is not satisfactory, better biomarkers for glaucoma are needed. The photopic negative response (PhNR) of the flash electroretinogram (ERG) is related to the activity of retinal ganglion cells and has been suggested to be a good indicator for the occurrence of glaucoma. Because the flash ERG is not sensitive to optic blur it might be superior to the pattern ERG and to imaging techniques. Based on indications that the flash colour plays a role, we propose a novel approach, namely to drive the PhNR with cone-isolating flashes using “silent substitution”. We propose to study how cone-driven signals in the stimulus and in the adapting background influence the PhNR of normal subjects and patients. The results of the PhNR will be compared with established electrophysiological, psychophysical and morphometric measures towards the ultimate aim of developing optimal stimuli for the PhNR to successfully diagnose glaucoma and to increase our understanding of the underlying pathophysiological mechanisms.
